Key Responsibilities of the RN Case Management - Travel- Deliver comprehensive case management services to assigned patients
- Coordinate with multidisciplinary teams for effective patient care
- Manage patient care plans and advocate for necessary health services
Requirements of the RN Case Management - Travel- Current RN license
- Minimum of 1 year acute care case management experience
- Familiarity with Meditech and Careport systems preferred
- CCM certification is advantageous
