Job Opportunity – Registered Nurse Case Manager – Colorado

Location: Englewood, Colorado
Compensation: Details provided to qualified applicants
Schedule: 8:00 AM – 4:30 PM, Monday-Friday or Friday-Monday rotation with weekend coverage

Requirements:
- Active Colorado RN license or ability to obtain
- Minimum 1 year of current Case Management RN experience (required)
- Discharge planning experience strongly preferred
- Emergency Department experience preferred
- Knowledge of palliative care and goals of care conversations is a plus
- Strong communication and interpersonal skills
- Adaptability to work across different units as needed
