Contract | 13-Week Travel Assignment | Days | 8:00 AM – 4:30 PM
Key Responsibilities
- Perform comprehensive case management assessments for hospitalized patients to evaluate clinical, psychosocial, and discharge planning needs
- Coordinate care transitions and discharge planning in collaboration with the interdisciplinary care team
- Facilitate utilization review and ensure appropriate level of care determinations throughout the patient stay
- Serve as a liaison between patients, families, physicians, and community resources to support continuity of care
- Document case management activities accurately and in accordance with facility and regulatory standards
Required Qualifications
- Active Registered Nurse (RN) license in Tennessee or a compact state license accepted in Tennessee
- Minimum of 2 years of RN experience, with experience in case management
- Strong knowledge of utilization management, discharge planning, and care coordination principles
- Ability to work independently in a fast-paced hospital environment
Preferred Experience / Skills
- Prior travel nursing experience in a case management or care coordination role
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred
- Familiarity with InterQual or Milliman care guidelines for utilization review
