**Job Description:**

- **Type:** Case Management
- **Location:** Englewood, CO
- **Number of Beds:** 20 beds per unit
- **Team:** Collaborative group of Case Managers and Social Workers
- **Shift Details:**
- Day Shift: Friday-Monday, 8:00 AM - 4:30 PM
- Day Shift: Monday-Friday, 8:00 AM - 4:30 PM
- **Patient Ratios:**
- Social Worker: 1:40
- RN: 1:20
- **Equipment:**
- Mobile devices and computers
- EMR: Meditech / CarePort
- **Typical Hiring Profile:**
- **Must Have:**
- Minimum 1-year current Case Management RN experience
- **Preferred:**
- Experience in Discharge Planning
- ED experience
- Knowledge of Palliative Care and Goals of Care Conversations
- **Typical Procedures:**
- Conducting Discharge Planning Assessments
- Facilitating Discharge Planning
