Key Responsibilities

• Perform case management assessments for patients on cardiac step-down (CPCU) and medical step-down (PCU) units
• Develop and coordinate individualized discharge plans, including post-acute placements and home-based cardiac rehab referrals
• Collaborate closely with consistent physician and mid-level provider teams to support patient care transitions
• Manage a caseload across two step-down units (CPCU - 14 beds, PCU - 14 beds) at a 1:14 staff-to-patient ratio
• Document and track case management activities using Meditech and Midas EMR platforms
• Operate effectively in a fast-paced, constantly changing environment requiring flexibility and adaptability

Required Qualifications

• Minimum 1 year of current acute care Case Manager RN experience
• Active Kansas (KS) RN licensure required
• BLS certification required

Preferred Experience / Skills

• Experience with Midas and HCA case management systems
• Prior experience in an HCA hospital setting
