The overall goal of the position is to enhance the quality of patient management and satisfaction. The role promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail. Adaptability to frequent change and compliance with regulatory and departmental guidelines and policies is essential. Essential Duties: Identify cases and prioritize daily work lists to highlight patients and new admissions. Conduct and document assessments and a plan of care following departmental guidelines. Participate in daily care management meetings as per established protocols. Consult with appropriate team members when indicated. Review and adjust discharge plans based on team discussions and updates. Attend and actively participate in case management meetings for assigned units. Alert the care team to any concerns that could impact anticipated patient discharge. Assist with identifying expected discharge dates. Monitor unique and high-risk cases needing escalation for further attention. Coordinate family meetings as necessary to support the progression of care. Provide education on community resources and coordinate referrals as necessary. Communicate medical milestones for transition with patients and families. Monitor all observation patients to ensure appropriate care progression. Assess discharge plans continually to determine post-discharge needs. Verify patient understanding and agreement with discharge plans. Consult with the social worker and/or utilization manager as needed.
