Maintain clear, accurate, and timely case management documentation.
Educate physicians and staff regarding utilization, level-of-care, and discharge planning issues.
Identify barriers to discharge and coordinate appropriate solutions with the care team.
Maintain compliance with applicable federal, state, payer, and hospital regulations.
Participate in departmental meetings, quality improvement initiatives, and continuing education.
