- Experience in inpatient case management
- Experience working at the bedside, preferably with at least 5 years of experience, ICU experience preferred

Certifications:
- BLS
Must-Have:
- Current experience with post-acute facility referral platforms
- Experience in helping to facilitate discharge planning
Description:
The role involves managing pediatric patient caseloads.
The position requires active collaboration with various clinical teams.

However, they should possess a solid understanding of case management principles.
The role requires flexibility to float to various pediatric units as needed.
