Description

Key Responsibilities
  • Provide case management services on an inpatient acute rehabilitation unit (26-bed floor) serving an IRF patient population
  • Manage a caseload of approximately half the unit census, coordinating care across the continuum
  • Support patients with common diagnoses including stroke, traumatic brain injury, hip fracture, trauma, acute renal failure, wound care, and amputation
  • Collaborate with interdisciplinary team members in a flexible, team-oriented environment
  • Document and manage patient information using EPIC EMR
  • Participate in weekend coverage as expected (approximately 1-5 weekend hours)
Required Qualifications
  • Active RN license required
  • Minimum 1 year of case management experience
  • No call requirements
  • No float requirements
Preferred Experience / Skills
  • Experience with EPIC EMR
  • Background working in an inpatient rehabilitation or acute care setting
  • Familiarity with IRF patient populations including neurological and trauma diagnoses
