Key Responsibilities

• Provide acute care case management services within a hospital setting
• Coordinate patient care plans, discharge planning, and utilization review activities
• Utilize clinical decision-support tools to assess medical necessity and appropriateness of care
• Collaborate with interdisciplinary teams to facilitate timely and effective patient transitions
• Document case management activities accurately within electronic health record systems

Required Qualifications

• Active RN license valid in the state of Florida (or compact eligibility)
• Minimum 2 years of acute (hospital) case management experience
• Proficiency with Meditech, Midas, and Interqual
• Ability to work every other weekend as required

Preferred Experience / Skills

• Experience in a large, multi-service acute care hospital environment
• Familiarity with utilization management and payer requirements
• Background working within a trauma or cardiovascular care setting
