Key Responsibilities
Provide acute care case management services within a hospital setting
Coordinate patient care plans, discharge planning, and utilization review activities
Utilize clinical decision-support tools to assess medical necessity and appropriateness of care
Collaborate with interdisciplinary teams to facilitate timely and effective patient transitions
Document case management activities accurately within electronic health record systems
Required Qualifications
Active RN license valid in the state of Florida (or compact eligibility)
Minimum 2 years of acute (hospital) case management experience
Proficiency with Meditech, Midas, and Interqual
Ability to work every other weekend as required
Preferred Experience / Skills
Experience in a large, multi-service acute care hospital environment
Familiarity with utilization management and payer requirements
Background working within a trauma or cardiovascular care setting
