**Job Description:**
- **Unit Information:**
- Case Management and Social Work Department
- Coverage of the entire hospital (not one specific unit)
- Total Staff: 22
- **Team Composition:**
- Day Shift:
- 10 Case Managers (CM)
- 3 Social Workers (SW)
- 2 CM Assistants
- 1 Manager
- 1 Director
- Night Shift: Not Applicable
- **Patient Ratios:**
- 20-25 patients/RN
- 50 patients/SW
- **Equipment and Technology:**
- Computer and phone provided
- EMR: Meditech/Meditech Expanse (effective 8/13)
- **Typical Hiring Profile:**
- **Must Have:**
- 2 years hospital case management experience as a registered nurse (RN)
- **Preferred:**
- Additional case management experience in a hospital setting
- **Responsibilities:**
- Discharge planning
- Confirming patient status using Interqual criteria
- **Best Personality Fit:**
- Flexible
- Comfortable asking questions
- **Required Certifications:**
- Valid RN license
- **Floating Area(s):**
- Any med-surge unit/ICU unit if needed
- **Dress Code:**
- Business casual attire
- Optional: Plain white lab coat
- **Weekends and On Call:**
- No weekend requirement
