Key Responsibilities- Provide hospital-based case management services within an acute care setting
- Coordinate patient care plans, discharge planning, and utilization review activities
- Collaborate with interdisciplinary teams to ensure appropriate and timely transitions of care
- Utilize MIDAS for documentation and case management workflows
- Assess patient needs and connect individuals with appropriate community and facility resources
Required Qualifications- Active Registered Nurse (RN) license in good standing
- Minimum of 2 years of case management experience in a hospital setting
- HCA system experience required
- MIDAS proficiency required
Preferred Experience / Skills- Experience with utilization management and payer authorization processes
- Familiarity with acute care discharge planning in a high-volume environment
- Case Management certification (CCM or ACM) is a plus
