Key Responsibilities
Provide hospital-based case management services within an acute care setting
Coordinate patient care plans, discharge planning, and utilization review activities
Collaborate with interdisciplinary teams to ensure appropriate and timely transitions of care
Utilize MIDAS for documentation and case management workflows
Assess patient needs and connect individuals with appropriate community and facility resources
Required Qualifications
Active Registered Nurse (RN) license in good standing
Minimum of 2 years of case management experience in a hospital setting
HCA system experience required
MIDAS proficiency required
Preferred Experience / Skills
Experience with utilization management and payer authorization processes
Familiarity with acute care discharge planning in a high-volume environment
Case Management certification (CCM or ACM) is a plus
