Job Title
RN Case Manager – Inpatient Rehabilitation (Travel Contract)
Location
Gainesville, GA | Acute Rehab
Schedule / Job Type
Contract | Days | 8:00 AM – 4:30 PM | No weekends | No call requirements | 13-week assignment
Key Responsibilities
- Provide case management services within a 40-bed inpatient acute rehabilitation unit
- Coordinate care for patients across a range of diagnoses including stroke, spinal cord injury, medical complex, and orthopedic conditions
- Utilize EPIC and UDS electronic health record systems to document and manage patient care
- Collaborate with an interdisciplinary team to support patient transitions and discharge planning
- Manage caseload with no float requirements and no call obligations
Required Qualifications
- Active RN licensure in good standing (or Licensed Social Worker – LSW)
- Minimum 2 years of case management experience
- Proficiency or ability to work within EPIC and UDS EHR platforms
- Strong communication and team collaboration skills
Preferred Experience / Skills
- Experience in an inpatient acute rehabilitation setting (not skilled nursing)
- Familiarity with rehabilitation patient populations including stroke, spinal cord injury, and orthopedic diagnoses
- Positive, team-oriented attitude with the ability to thrive in a fast-paced environment
