Key Responsibilities
Conduct face-to-face interviews with patients and families to assess needs and develop discharge plans
Perform InterQual reviews on every patient, including Observation reviews and DPEs
Participate in multidisciplinary rounds (MDRs) on each unit daily
Manage a caseload with an expected patient ratio of 1:40 Monday-Friday
Float throughout the entire hospital, including Women & Children's Health and the Emergency Department as needed
Document and manage case activity using MediTech and Midas platforms
Required Qualifications
Active Registered Nurse (RN) license
Minimum 1 year of Case Management experience
Experience with Midas and InterQual
Licensed Master's degree (required)
No on-call requirements
Preferred Experience / Skills
Knowledge of local and regional community resources
NATE certification or familiarity
Comfortable working independently and collaboratively across multiple units and departments
