Key Responsibilities

• Conduct face-to-face interviews with patients and families to assess needs and develop discharge plans
• Perform InterQual reviews on every patient, including Observation reviews and DPEs
• Participate in multidisciplinary rounds (MDRs) on each unit daily
• Manage a caseload with an expected patient ratio of 1:40 Monday-Friday
• Float throughout the entire hospital, including Women & Children's Health and the Emergency Department as needed
• Document and manage case activity using MediTech and Midas platforms

Required Qualifications

• Active Registered Nurse (RN) license
• Minimum 1 year of Case Management experience
• Experience with Midas and InterQual
• Licensed Master's degree (required)
• No on-call requirements

Preferred Experience / Skills

• Knowledge of local and regional community resources
• NATE certification or familiarity
• Comfortable working independently and collaboratively across multiple units and departments
