Requirements:- Active and unencumbered Registered Nurse (RN) license in Texas or compact state license
- Minimum of 1 year acute care case management experience required
- Strong knowledge of discharge planning, utilization review, and care coordination
- Experience with Meditech and Careport EMR systems preferred
- Certified Case Manager (CCM) credential is a plus
- Self-starter with solution-focused approach and collaborative mindset
