TravelNurseSource is working with Malone Healthcare to find a qualified Case Manager RN in Englewood, Colorado, 80110!
Key Responsibilities- Manage a caseload of approximately 1:20 patients as an RN Case Manager across inpatient units of approximately 20 beds each
- Conduct discharge planning assessments and coordinate comprehensive discharge planning for assigned patients
- Collaborate with a multidisciplinary team including social workers and other case management staff to support patient transitions
- Utilize electronic medical record systems (Meditech and CarePort) and mobile technology to document and coordinate care
- Adapt effectively across different units as patient and staffing needs require
Required Qualifications- Active RN license required
- Minimum 1 year of current Case Management RN experience
- Strong communication skills and professional, collaborative approach
- Ability to adapt to different unit environments and patient populations
Preferred Experience / Skills- Experience with discharge planning in an acute care setting
- Emergency Department (ED) experience
- Palliative care knowledge and goals-of-care conversation experience
