Description

Key Responsibilities
  • Manage a caseload of approximately 1:20 patients as an RN Case Manager across inpatient units of approximately 20 beds each
  • Conduct discharge planning assessments and coordinate comprehensive discharge planning for assigned patients
  • Collaborate with a multidisciplinary team including social workers and other case management staff to support patient transitions
  • Utilize electronic medical record systems (Meditech and CarePort) and mobile technology to document and coordinate care
  • Adapt effectively across different units as patient and staffing needs require
Required Qualifications
  • Active RN license required
  • Minimum 1 year of current Case Management RN experience
  • Strong communication skills and professional, collaborative approach
  • Ability to adapt to different unit environments and patient populations
Preferred Experience / Skills
  • Experience with discharge planning in an acute care setting
  • Emergency Department (ED) experience
  • Palliative care knowledge and goals-of-care conversation experience
