Port Charlotte, FL | Acute Care

Schedule / Job Type

Contract | 13-Week Travel Assignment | Days, 8:00 AM – 4:30 PM | Expected schedule includes every other weekend and holidays in rotation

Key Responsibilities

• Provide unit-based RN Case Management services within an acute care hospital setting
• Manage a caseload of 20–25 patients per RN Case Manager
• Coordinate care planning, discharge planning, and utilization review for assigned patients
• Collaborate with social work staff and interdisciplinary team members to facilitate appropriate patient transitions
• Document case management activities accurately within the facility EMR system (Meditech)

Required Qualifications

• Active Registered Nurse (RN) license valid in the state of Florida
• Minimum 1 year of hospital-based Case Management experience required
• Candidates without hospital Case Management experience will not be considered

Preferred Experience / Skills

• Experience with InterQual criteria and utilization review processes
• Prior experience with Meditech EMR
• Detail-oriented with strong time management and multitasking abilities
