Key Responsibilities- Provide case management services on a 40-bed Neuro Step Down unit
- Manage a patient caseload with ratios of approximately 1:20-30
- Collaborate with social work staff and interdisciplinary team members to coordinate patient care and discharge planning
- Utilize EMR systems including Meditech and Careport to document and track patient progress
- Float to varied case management assignments as needed
Required Qualifications- Active RN license required
- Minimum of 1 year of acute care case management experience
- Travel nursing candidates considered for this role
Preferred Experience / Skills- Prior experience with Meditech and/or Careport EMR systems
- Certified Case Manager (CCM) credential
- Self-starter with a solution-focused and collaborative approach to patient care
