Key Responsibilities
Provide case management services on a 40-bed Neuro Step Down unit
Manage a patient caseload with ratios of approximately 1:20-30
Collaborate with social work staff and interdisciplinary team members to coordinate patient care and discharge planning
Utilize EMR systems including Meditech and Careport to document and track patient progress
Float to varied case management assignments as needed
Required Qualifications
Active RN license required
Minimum of 1 year of acute care case management experience
Travel nursing candidates considered for this role
Preferred Experience / Skills
Prior experience with Meditech and/or Careport EMR systems
Certified Case Manager (CCM) credential
Self-starter with a solution-focused and collaborative approach to patient care
