Description:
We are seeking contract nurses to support rising nursing caseloads.
The position will involve conducting nursing assessments and collaborating with external agencies to ensure appropriate services are delivered.
The assessments will be used to authorize skilled nursing services for members.
The successful candidate will conduct in-person visits statewide and have a core district they serve.
Responsibilities include monitoring health needs, assisting with referrals, and advocating for members' rights.
The Registered Nurse will complete a report for each member, documenting the medical necessity for skilled services that meet individualized needs.
In-person assessments will be conducted every 90 days and as needed.
This is a field-based position, and travel is required to complete in-person assessments.
The role will involve completing nursing referrals and vendor calls.
Special projects may be assigned as necessary.
Documentation must be completed within established timeframes.
Collaboration will occur internally and externally, participating in multidisciplinary planning meetings.
The nurse will work to determine and identify appropriate services per established policies and guidelines.
Collaboration with members and their support systems is essential.
Mandatory meetings will be attended, and required annual trainings must be completed.
