Job Title: Registered Nurse Profession: Nursing Specialty: Staff Augmentation Duration: Contract Position Shift: Day Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of 2 years of recent RN experience working with individuals in home health, hospice, long-term care, or case management. License: Registered Nurse licensed to practice in the Client, active and in good standing without restrictions. Certifications: Additional training or certification in behavioral health, complex medical conditions, or nursing case management preferred. Must-Have: Excellent communication skills, including advanced abilities in explaining complex information to members and support systems. Proficient with Google Office, Microsoft Office, and Adobe Acrobat, including instant message features and document uploading. Description: We are seeking the support of contract nurses to address rising nursing caseloads. The position involves collaborating with external agencies to ensure appropriate services are delivered. Conducting nursing assessments is a key function of this role. Assessments are used to authorize skilled nursing services for members. Successful candidates will conduct in-person visits statewide and monitor health needs. They will assist with referrals and advocate for member rights. Nurses will complete reports documenting the medical necessity for skilled services that meet individualized needs. In-person assessments are required every 90 days and as needed. This is a field-based position, and travel is required. The role includes completing nursing referrals and placing vendor calls. Nurses may complete special projects as needed. Documentation must be completed within the timeframe established. Interprofessional collaboration is essential both internally and externally. Participation in multidisciplinary planning meetings is required to determine appropriate services per established policies and guidelines. Collaboration with members and their support systems is crucial. Attending mandatory meetings and completing required annual training is expected.
