Experience: Minimum of 2 years of recent RN experience working with individuals in home health, hospice, long-term care, or case management.
License: Registered Nurse licensed to practice in the Client, active and in good standing without restrictions.
Certifications: Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred.
Must-Have: Excellent communication skills, including advanced abilities in explaining complex information to members and their support systems. Proficiency with Google Office, Microsoft Office, and Adobe Acrobat, including document uploading and a willingness to learn new software.
Description: We are seeking contract nurses to support the rising nursing caseloads currently being experienced.
Locations will vary based on the assignment.
The selected candidate will act as a floater, stepping in for nurses on annual leave, sick leave, or other types of leave.
Key duties include conducting nursing assessments and collaborating with external agencies to ensure services are delivered appropriately.
Assessments will be used to authorize skilled nursing services for members.
In-person visits statewide are required, with a core district designated for the nurse.
Responsibilities include monitoring health needs, assisting with referrals, and advocating for members' rights.
The Registered Nurse will complete reports documenting the medical necessity for skilled services needed by members.
In-person assessments will take place every 90 days and as required.
The position is field-based, necessitating travel for in-person assessments.
Nursing referrals and vendor calls must be completed as needed.
Successful candidates will engage in multidisciplinary planning meetings to identify appropriate services per established policies and guidelines.
Collaboration with members and their support systems is essential.
Participation in mandatory meetings and completion of required annual training is expected.
