Seeking an experienced Ambulatory RN/OB Case Manager to provide comprehensive, patient-centered nursing care in an outpatient OB clinic. The RN will coordinate care, manage referrals and follow-ups, educate patients and families, collaborate with OB providers and interdisciplinary teams, and help ensure continuity of care throughout the patient's treatment journey.
Requirements:
- Active, unrestricted Alaska RN license required at time of submission
- OB Case Management experience required
- BLS required
- Case Management certification preferred
- Graduate of an accredited nursing program
- BSN preferred
Key Responsibilities:
- Perform comprehensive patient assessments and develop individualized care plans
- Coordinate OB patient care, referrals and follow-up services
- Collaborate with providers and interdisciplinary teams
- Provide patient and family education
- Monitor patient needs and escalate changes in condition appropriately
- Assist patients with navigating healthcare services
- Administer medications and perform clinic-based nursing procedures as ordered
- Maintain accurate and timely EHR documentation
- Promote patient safety, quality care and culturally responsive services
- Participate in required training, competencies and quality-improvement activities
