Med Ninjas is seeking a travel nurse RN Case Management for a travel nursing job in Chula Vista, California.
Position Overview:
The Registered Nurse Case Manager coordinates patient care, discharge planning, and healthcare resources to achieve optimal patient outcomes. Candidates must have inpatient hospital experience and an active California RN license.
Key Responsibilities:
Assess patients’ medical needs, identify care goals, and develop individualized care plans with patients, families, and healthcare teams.
Coordinate care transitions, referrals, discharge planning, and appropriate healthcare resources.
Monitor patient progress, evaluate outcomes, and modify care plans based on changing clinical needs.
Educate patients and families on treatment plans, medications, follow-up appointments, and discharge instructions.
Collaborate with physicians, social workers, and interdisciplinary teams to address barriers to care.
Advocate for patients’ rights, safety, and access to appropriate healthcare services.
Maintain accurate clinical documentation and participate in patient care conferences.
Coordinate referrals and follow-up services to support continuity of care.
Required Certifications & Licenses:
Active California RN License
CPR/BLS Certification
Location: Chula Vista, California
Experience Required: Inpatient hospital experience is mandatory.
