WissenCare is seeking a travel nurse RN Case Management for a travel nursing job in Fairfield, California.
Job Description:
The Inpatient Case Manager assess, plan, implement, monitor & evaluate options & services to affect an appropriate, individualized plan for hospital patient care across the continuum of care.
The Inpt CM will follow assigned patients working w/team members & Social Services to prioritize & strategize each day’s activities.
Using independent judgment & discretion, the patient must be assessed & reassessed as an ongoing process to address the patient’s broad spectrum of needs.
Education and Experience:
Graduate of an accredited nursing school or college.
Minimum of 2 years acute clinical nursing experience preferred.
Case Management experience strongly preferred.
BSN/MSN preferred
Licensure:
Current California state RN license. Certified Case Manager (CCM) or Accredited Case Manager (ACM)
