Travel RN – Case Management | La Mesa, CA
We have a 13-week Travel RN Case Management contract available in La Mesa, California.
Day Shift – 5x8s (8:00 AM–4:30 PM)
Every other weekend required
Requirements:
- Active California RN License
- CPR
- Minimum 3 years of recent RN experience
- Minimum 2 years of recent experience in Case Management/specialty area
- Associate Degree or higher
Responsibilities:
- Assess patient and family needs and develop individualized care plans
- Coordinate patient care with interdisciplinary healthcare teams
- Monitor patient progress and update care plans as needed
- Facilitate communication, referrals, and continuity of care
- Provide patient and family education
- Advocate for patients and families throughout the care process
- Maintain accurate clinical documentation
- Support safe and efficient patient outcomes and discharge planning
