Case Manager Registered Nurse (RN)
Job Description
We are seeking a dedicated and skilled Case Manager Registered Nurse (RN) to join our team. This role is essential in coordinating patient care to ensure optimal utilization of resources while maintaining high standards of quality and patient satisfaction.
Key Responsibilities
- Conduct thorough assessments of patients' medical histories and needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Monitor patient progress and adjust care plans as necessary.
- Act as a liaison between patients, families, and medical professionals.
- Document patient care activities accurately in the electronic medical record system (Epic).
- Provide support and education to patients and their families about healthcare options and resources.
- Ensure compliance with regulations and standards of care.
Qualifications
- Active Registered Nurse (RN) license in the state of Alaska.
- Minimum of 1 year of nursing experience, with a focus on acute care.
- Experience with Epic electronic health record system.
- Basic Life Support (BLS) certification required; Additional certifications such as ACM are preferred.
- Availability to work weekends on a rotating schedule (every other weekend or four weekend days in a four-week period).
Contract Details
- Contract lengths of 13 or 26 weeks available.
- Shift options include days structured as 4x10 or 5x8 hour schedules.
- Shift times may vary; typical shifts include 07:00 - 15:30, 07:30 - 16:00, 08:00 - 16:30, and others.
- Guaranteed hours: 72 hours per two weeks, with the possibility of being called off for one 8-hour shift per two weeks.
- Only one time off request of five days or less is permitted per 13-week assignment. No holiday time off during the auto offer period.
Additional Information
- Position may require floating within the scope of practice to meet facility needs.
- Local candidates will not be accepted; candidates must live at least 50 miles away from the facility.
