Case Manager Registered Nurse (RN)

We are seeking a skilled and dedicated Case Manager Registered Nurse (RN) to join our Case Management team. The ideal candidate will possess a strong background in acute care and experience with Epic charting, ensuring high-quality patient care and effective management of case loads.

Job Details

• Shift: Day 4x10 or 5x8
• Contract Length: 13 or 26 Weeks
• Scheduled Hours:
  • 0700-1530
  • 0730-1600
  • 0800-1630
  • 0830-1700
  • 0700-1730
  • 0730-1800
• Weekend Requirement: Yes - every other weekend or 4 weekend days in a 4-week schedule
• Guaranteed Hours: 72 Hours per 2 weeks, with the possibility of being called off for 1 shift per 2 weeks

Responsibilities

• Manage patient cases to ensure effective care coordination and resource utilization.
• Collaborate with healthcare team members to develop and implement care plans.
• Utilize Epic for documentation and patient management tasks.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on treatment options and care processes.

Qualifications

• Must possess an active Alaska RN license.
• Minimum of 1 year of acute care experience required.
• BLS certification is mandatory; ACM certification is preferred.
• Ability to float within scope to meet facility needs as required.
• Local candidates must reside at least 50 miles from the facility.

• No block scheduling options available.
• RTO restrictions apply: only 1 time off request per 13-week assignment of 5 days or less; no holiday RTO for Auto Offer.
