Case Manager Registered Nurse (RN)

We are seeking a qualified Case Manager Registered Nurse (RN) to join our dynamic healthcare team. This position is essential in managing patient care and ensuring a seamless flow of healthcare services. The successful candidate will demonstrate their ability to coordinate care and advocate for patients throughout their healthcare journey.

Key Responsibilities

• Manage and coordinate patient care effectively, ensuring optimal health outcomes.
• Conduct assessments and develop care plans tailored to individual patient needs.
• Collaborate with healthcare providers and support staff to facilitate comprehensive care.
• Utilize electronic health record systems, specifically Epic, for documentation and patient management.
• Provide education and support to patients and families regarding treatment plans and resources.
• Monitor patient progress and adjust care plans as necessary, responding to patient needs in a timely manner.

Qualifications

• Current Registered Nurse (RN) license in Alaska is required.
• Minimum of 1 year of experience in a clinical setting, preferably in acute care.
• Basic Life Support (BLS) certification is required; ACM certification is preferred.
• Proven experience with Epic electronic medical records is required.
• Strong communication and organizational skills, with the ability to work collaboratively in a team-oriented environment.

Schedule

This position offers flexible scheduling options, including either 4x10 or 5x8 hour shifts during the day. Weekend availability is expected, with every other weekend required or 4 weekend days within a 4-week schedule.

Additional Information

• No on-call responsibilities required.
• Guaranteed minimum of 72 hours per 2 weeks; traveler may be called off for one 8-hour shift every two weeks.
• Local candidates may not apply; applicants must reside at least 50 miles from the facility.
