Seeking an experienced RN Case Manager with Home Health Case Management experience for a 13-week day shift assignment. Responsible for coordinating patient care, managing caseloads, developing care plans, and collaborating with interdisciplinary teams to ensure quality outcomes and efficient resource utilization. Epic experience is preferred.

Assignment Details:

  • Specialty: RN Case Manager (Home Health)
  • Shift: 5x8 Days (8:00 AM - 4:30 PM)
  • Hours: 40/week
  • Contract: 13 Weeks
  • Start Date: 10/05/2026
  • Openings: 1
  • Charting: Epic
  • Requirements: Active CA RN License, BLS, and Home Health Case Management experience required. Must meet all applicable compliance and credentialing requirements.
