Case Manager

Job Summary

A Case Manager is responsible for effective discharge planning and patient referrals within an inpatient care setting. The role entails collaborating with an interdisciplinary team, coordinating patient care, and ensuring a smooth transition for patients upon discharge.

Key Responsibilities

• Perform discharge planning and develop plans for patient referrals.
• Work closely with social workers and medical teams on the unit.
• Round with physicians and team members to discuss patient care and progress.
• Manage a caseload of 17-25 patients, ensuring comprehensive care delivery.
• Assist in arranging resources such as housing or medical referrals, including KCI referrals for wound vacs.

Qualifications

• Minimum of 1 year of inpatient case management experience required.
• Preferred experience in critical care settings.
• Knowledge of discharge planning processes and referral systems.
• Familiarity with CarePort, MCG, and/or InterQual is preferred.
• BLS certification is required.
• Completion of PRI & SCREEN certifications, which can be obtained online, is preferred.

Work Schedule

Normal working hours are Monday through Friday, from 8:00 AM to 4:30 PM, with breaks in accordance with facility policy. Weekend or holiday coverage may be required occasionally with manager approval.

Orientation and Training

All new hires will undergo a two-day hospital orientation, which includes EMR training, followed by 1-2 weeks of training with a preceptor based on individual competency progression.

Dress Code

Business casual attire or scrubs in neutral colors is acceptable, excluding royal blue or maroon. A lab coat will be provided.
