Job Requirements

Minimum 2 years of nursing experience.
Active California Registered Nurse (RN) license required.
Current BLS certification required.
Strong communication, critical thinking, and organizational skills.
Ability to collaborate with physicians, bedside RNs, utilization management, medical social workers, and interdisciplinary teams.
Knowledge of discharge planning, resource coordination, and appropriate patient placement.
Ability to assess patient needs and coordinate transitions to the appropriate level of care.
Understanding of healthcare resource utilization and strategies to prevent unnecessary hospital admissions and readmissions.
Ability to coordinate with patients, families, insurance providers, community resources, and other healthcare professional
