• Must have Home Health Case Manager experience — this is specifically required
• Experience with Home Health
• Experience with care coordination
• Experience with discharge planning
• Experience with disease management
• Previous Charge experience required
• Strong experience managing a patient caseload and coordinating interdisciplinary care

Preferred

• Epic experience
• Experience working with geriatric patients
• Experience coordinating with PT, social services, transportation, and other interdisciplinary teams
