Case Manager – Acute Care Hospital

Location: Mission Hills, CA
Contract: 26 weeks 
Shift: Days, 5×8 — 8:00 AM–4:30 PM
Setting: Acute Care Hospital
Ratio: 1:20
Floating: No
First-time travelers: Yes — clinical will screen first-time travelers
 

Must-Have Experience

  • 2+ years of acute care hospital Case Manager experience
  • Hospital experience is a HARD requirement
  • Experience must include:
    • Care Coordination
    • Discharge Planning
    • Patient assessment
    • Treatment-plan participation
    • Family meetings
  • Epic experience required
  • Experience using InterQual criteria
  • Experience with HMOs/IPAs
  • Experience arranging DME, SNF placement, and ambulance transportation independently
  • Knowledge of Medicare/CMS
  • CA license required; pending license accepted only if licensed by start date
  • Travel experience required per submission requirements
