RN Case Manager

Job Description

We are seeking a dedicated RN Case Manager to manage an acute care caseload of medical patients. The ideal candidate will possess a strong clinical background and be able to effectively coordinate patient care. This position requires a minimum of two years of experience in a similar role.

Key Responsibilities

• Manage acute care caseloads of medical patients.
• Coordinate patient care and ensure effective communication among healthcare team members.
• Assess patient needs and develop individualized care plans.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to patients and families regarding health conditions and treatment options.

Qualifications

• Minimum of 2 years of experience in case management or related field.
• Active nursing license (RN) in the applicable state.
• General certifications such as ACLS, BLS/BCLS, PALS, NRP, ATT are required.
• Nursing certifications including CPI, ENPC, RNFA, TNCC, AWHONN, NIHSS, STABLE, ASLS are preferred.

Work Schedule

This position primarily operates during the day, with possible weekend and holiday coverage required.
