Hospital Case Manager RN 


Job Requirements

  • Minimum 2 years of acute care hospital Case Manager experience.
  • Hospital experience is a must.
  • Experience performing Care Coordination and Discharge Planning.
  • Epic charting experience required.
  • Active California RN license required.
  • Pending license accepted, but must be licensed by start date.
  • Travel experience required according to the job requirements.
  • First-time travelers accepted; clinical will screen first-time travelers.
  • Must be able to work weekend rotation.
  • Must commit to working 3 of the 5 holidays.

Schedule

  • Shift: Day
  • Schedule: 5×8-hour shifts per week
  • Hours: 8:00 AM – 4:30 PM
  • Weekend: Weekend rotation and meet department needs
  • Call: Not specified
  • Floating: No scheduled float listed
  • Orientation: 40 hours
  • Patient Ratio: Approximately 1:20
  • Scrub: Scrubs or business attire

Responsibilities & Required Skills

  • Assess patients and participate in the development of treatment plans.
  • Perform Care Coordination and Discharge Planning.
  • Utilize InterQual Admission Criteria (ED only).
  • Knowledge of CMS (Centers for Medicare and Medicaid Services).
  • Knowledge of DRG (Diagnosis Related Groups).
  • Knowledge of Plan of Care.
  • Experience arranging DME, SNF placement, and ambulance transportation independently.
  • Experience working with HMOs, IPAs, and other healthcare organizations.
  • Ability to organize and participate in family meetings.
  • Knowledge of drug/alcohol abuse treatment options.
  • Experience supporting patients experiencing homelessness.
  • Knowledge of patients with mental health history or in crisis.
  • Knowledge of advance directives, POLST, and protective services processes.
  • Computerized documentation required.
  • Proficient in Microsoft applications.
