Med Ninjas is seeking a travel nurse RN Case Management for a travel nursing job in Sunnyvale, California.
Job Summary (List Format):
- Admit patients to hospice service and assess physical, psychosocial, environmental, safety, and developmental needs of patients and families
- Develop individualized plans of care based on thorough assessments
- Document observations and interventions related to the patient’s environment
- Coordinate care with a multidisciplinary team
- Participate in the hospice program’s quality assessment and performance improvement initiatives
- Conduct follow-up visits as needed
- Serve patients located in Oakland, Alameda, San Leandro, Ashland, Castro Valley, San Lorenzo, Hayward, Union City, Fremont, Newark, Milpitas, and neighboring cities
- Requirement: Minimum of 1 year of hospice nursing experience
Med Ninjas Job ID #22620. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
