RN Case Manager

Join our team as a Registered Nurse (RN) Case Manager, focused on providing exceptional care management and discharge planning for patients. This position requires a commitment to working Monday through Friday, including every third weekend and holidays as needed.

Position Information

Role: RN Case Manager
Specialty: Case Management, Discharge Planning

Weekend Requirements: Every other weekend availability, including holidays
Float Requirements: Ability to cover department needs as required

Key Responsibilities

Conduct discharge planning and care coordination for patients
Collaborate with healthcare teams to facilitate patient care
Manage complex and high-acuity patient cases, with a focus on behavioral health

Qualifications

Experience: Minimum of 5 years in hospital case management and 5+ years in acute adult care nursing
Must Have Skills: Expertise in discharge planning and care coordination
Preferred Experience: Over 5 years of nursing practice in a hospital setting

Personality Fit

The ideal candidate should possess a flexible, easy-going demeanor and exhibit strong problem-solving skills. A calm attitude and a can-do mindset are essential for success in this role.
