Position Overview

We are seeking an experienced Registered Nurse (RN) specializing in Case Management for a travel assignment at Mission Hospital in Asheville, North Carolina. Key Responsibilities

• Coordinate patient care and discharge planning across multiple hospital units.
• Collaborate closely with RN Case Managers, physicians, social workers, nursing staff, and other healthcare professionals.
• Assist with identifying patient needs, discharge barriers, and appropriate post-acute care resources.
• Facilitate communication between patients, families, and multidisciplinary care teams.
• Maintain accurate and timely documentation within hospital electronic medical record systems.
• Support safe and efficient patient transitions between levels of care.
• Manage multiple patient cases while prioritizing tasks in a fast-paced hospital environment.
• Build trusting relationships with patients, families, and healthcare team members.
• Assist with coordinating post-acute services and ensuring continuity of care.

Qualifications

Required:

• Active Registered Nurse (RN) license with authorization to practice in North Carolina.
• Previous hospital-based RN Case Management experience.
• Strong clinical documentation and organizational skills.
• Excellent verbal and written communication abilities.
• Ability to manage competing priorities and shift attention between tasks.
• Demonstrated ability to collaborate with multidisciplinary healthcare teams.
• Strong interpersonal skills and ability to establish trust with patients and families.

Preferred:

• Previous travel nursing experience.
• Experience using Cerner, NATE, or Ensocare.
• Experience coordinating discharge planning and post-acute care services.
• Experience in a large acute-care hospital environment.
