Case Manager Registered Nurse (RN)

Job Summary

We are seeking an experienced Case Manager Registered Nurse (RN) or Social Worker who will be responsible for coordinating patient care and discharge planning in a fast-paced acute hospital setting. This position requires the ability to manage a substantial patient caseload effectively.

Key Responsibilities

• Oversee discharge planning and ensure successful transitions of care for patients.
• Manage a patient assignment of up to 20 individuals per day.
• Coordinate services across multiple medical units, including ICU, CCU, Medical Surgical, and Women's Services.
• Collaborate with healthcare providers and family members to develop comprehensive care plans.
• Provide support for patients during their hospital stay, addressing both medical and psychosocial needs.
• Participate in multidisciplinary team meetings and ensure adherence to best practices in case management.

Qualifications

• Minimum of 2 years of experience in a similar role, preferably in an acute care hospital.
• Experience in handling large case loads and conducting discharge planning.
• BLS certification is required.
• Current Maryland RN license is required.
• Travel experience is required.

Work Schedule

This position requires a standard work week of 5 days, with shifts from 8:00 AM to 4:30 PM. Candidates must be able to work every other weekend and every other holiday. Flexibility to float between medical units, ICU/CCU, and Maternal Child is also required.

Additional Requirements

All employees must be vaccinated for the flu prior to starting the position.

Preferred Qualifications

• Experience in acute hospital settings, including specific knowledge of ICU, Medical-Surgical, and other related units.
• Experience in case management or social work with a focus on navigating complex patient needs.
