Prime Staffing is seeking a travel nurse RN Case Management for a travel nursing job in Morristown, New Jersey.

We are seeking an experienced Registered Nurse Case Manager to coordinate patients throughout their acute-care hospitalization and manage the full discharge planning and transition-of-care process. The RN Case Manager will collaborate with patients, families, providers, healthcare teams, and post-acute facilities to support safe and appropriate transitions from the hospital to the next level of care.
Assignment Details
- Position: RN Inpatient Case Manager
- Schedule: 8:00 AM – 4:00 PM
- Weekend Requirement: Rotating weekends, every 4th or 5th weekend
- Holiday Requirement: One summer holiday and one winter holiday
- Setting: Acute Inpatient Hospital
Key Responsibilities
- Coordinate patients throughout their acute-care hospitalization.
- Manage all aspects of the discharge planning and transition-of-care process.
- Assess discharge needs and coordinate appropriate post-acute services.
- Coordinate patient placement in rehabilitation facilities.
- Support transitions to appropriate levels of care.
- Perform utilization management and quality screenings.
- Collaborate with physicians, interdisciplinary teams, patients, families, and post-acute providers.
- Support appropriate care progression and safe discharge planning.
- Coordinate care for subacute patients as required.
Required Qualifications
- Minimum 2 years of acute inpatient hospital discharge planning experience
- Previous inpatient hospital Case Management experience required
- Hospital experience required
- Experience with discharge planning and rehabilitation facility placement
- Subacute patient experience required
- Case Management experience
- Managed Care experience
- BSN required
- Active RN license required
Preferred Qualifications
- CCMC/CCM certification preferred
- Strong Utilization Management experience
- Experience coordinating complex transitions of care
- Strong communication and interdisciplinary collaboration skills
