RN Care Manager

Position Overview

We are seeking a dedicated RN Care Manager to join our care management team. This position is responsible for ensuring efficient discharge planning and facilitating care coordination for patients.

Work Schedule

• Monday through Friday
• Occasional weekend hours available, not required

Key Responsibilities

• Perform comprehensive discharge planning for patients.
• Coordinate patient care effectively across various settings.
• Collaborate with multidisciplinary teams to meet patient needs.
• Support the Care Management department and assist with coverage as needed.

Qualifications

• Experience: Minimum of 5 years in nursing with a focus on discharge planning and care coordination.
• Skills: Strong organizational and problem-solving abilities. Must possess a calm demeanor and a flexible, can-do attitude.

Patient Care

The RN Care Manager will work with a patient ratio of 1:20-24, handling various patient cases requiring effective management and care coordination.

Dress Code

• Scrubs or professional casual attire acceptable.
