RN-Care Manager
Job Summary
The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness through integrated functions of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail, adaptable to frequent changes, and compliant with regulatory and departmental guidelines and policies.
Essential Duties
- Identify cases and prioritize daily tasks by reviewing the work list to highlight patients and identify new admissions.
- Conduct and document assessments and develop care plans in Epic™ per departmental guidelines.
- Participate in Daily Care Management Touchpoints following established protocols.
- Consult Social Workers as per established criteria.
- Communicate with the Care Management Assistant (CMA) to share priorities as indicated.
- Attend and actively participate in CAPP meetings for assigned units to provide and receive updates on patient progression.
- Alert the care team to concerns that could impact patient discharge and assist with discharge readiness.
- Modify discharge plans based on information shared during meetings.
- Assist in identifying the expected discharge date (EDD).
- Complete follow-ups from CAPP meetings as appropriate.
- Attend weekly Complex Care Meetings (CCM) and present on patients, collaborating to resolve issues with complex patients and identifying trends.
- Formulate potential solutions with the Utilization Manager and Social Worker, continuously monitoring cases and following up on action items.
- Proactively identify high-risk cases needing escalation that are not scheduled for discussion that week.
- Discuss barriers to discharge and psychosocial concerns impacting care progression or readmission risk with the multidisciplinary team.
- Coordinate family meetings, as necessary, to support ongoing care.
- Provide education on community resources, support groups, and other appropriate resources to patients, families, and the care team.
- Educate and/or coordinate referrals to community resources and post-acute providers as required.
- Communicate medical milestones for patient transitions with patients and families.
- Monitor observation patients consistently to ensure appropriate progress in care.
- Assess discharge plans to determine post-discharge needs and communicate with patients, families, and the care team.
- Identify required authorizations for post-discharge services and refer to appropriate providers.
- Participate in medication resource management for non-resourced patients, as needed.
- Verify patient understanding and agreement of the discharge plan.
- Refer administrative tasks to the Care Management Assistant as necessary.
- Consult with Social Workers and/or Utilization Coordinators per established departmental protocols.
Required Skills & Experience
- Two (2) years of healthcare experience as a Registered Nurse.
- Strong assessment and critical thinking skills.
Required Education
- Nursing Diploma or ADN/ASN from an accredited school of Nursing.
Preferred Education
- BSN or MSN from an accredited school of Nursing.
Required Certifications & Licensure
- Licensed to practice as a Registered Nurse in the state of North Carolina.
