Care Manager
Job Summary
The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction, promote continuity of care, and ensure cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must possess strong organizational skills, attention to detail, adaptability to frequent changes, and compliance with regulatory and departmental guidelines and policies.
Essential Duties
- Identify cases and prioritize daily tasks by reviewing the work list and recognizing new admissions.
- Conduct and document assessments and a plan of care using Epic™ per departmental guidelines.
- Participate in daily care management touchpoints following established protocols.
- Consult with social workers based on established criteria.
- If needed, communicate with the Care Management Assistant to share priorities.
- Attend and actively participate in CAPP meetings for assigned units, providing and receiving information on patient progression.
- Notify the care team of any concerns affecting anticipated patient discharge and assist with care that promotes discharge readiness.
- Modify discharge plans based on information gathered during meetings.
- Assist in identifying the expected discharge date (EDD).
- Complete follow-up actions from CAPP meetings as appropriate.
- Attend weekly Complex Care Meetings and present on patients, collaborating to identify and resolve issues with complex cases.
- Monitor cases and follow up on all action items as needed.
- Proactively identify high-risk cases that require escalation.
- Discuss barriers to discharge and psychosocial concerns with multidisciplinary team members.
- Coordinate family meetings as necessary to facilitate care progression.
- Provide education on community resources, support groups, and other relevant resources to patients, families, and care teams.
- Educate and/or coordinate referrals to community resources and post-acute providers as needed.
- Communicate medical milestones for transition with patients and families.
- Identify patients with discharge barriers based on CAPP and Complex Care Meetings.
- Monitor observation patients for appropriate care progression throughout the day.
- Assess discharge plans and communicate post-discharge needs to the patient and care team.
- Identify required authorizations for post-discharge services and refer to appropriate providers.
- Participate in medication resource management for patients without resources, as needed.
- Verify patient understanding and agreement of the discharge plan.
- Refer administrative tasks to the Care Management Assistant.
- Consult with social workers or utilization managers as per departmental protocol.
Required Skills & Experience
- Two (2) years of health care experience as a Registered Nurse.
- Strong assessment and critical thinking skills.
Required Education
- Nursing Diploma or
- ADN or ASN from an accredited School of Nursing.
Preferred Education
- BSN or MSN from an accredited School of Nursing.
Required Certifications & Licensure
- Licensed to practice as a Registered Nurse in the state of North Carolina.
