Registered Nurse / Case Manager

We are seeking a dedicated Registered Nurse with Case Management experience to join our team. This position focuses on providing high-quality care in a fast-paced environment.

Shift and Schedule

• 5x8 schedule; Monday–Friday from 8:30 AM to 5:00 PM
• 40 hours per week

Weekend and Holiday Requirements

• Agency staff may be scheduled for every other weekend based on staffing needs.
• One holiday per year is required, aligned with staffing expectations.

Floating Requirements

Candidates may need to float within the scope to satisfy facility needs, primarily within the Emergency Department and medical inpatient units.

Time Off Restrictions

• One pre-approved time-off request is allowed per contract.
• The system automatically approves 14 days or less of time off.
• Seasonal holidays (winter and summer) require attendance, with no time off permitted.
• No time off is granted on hard-to-fill days such as Easter, Mother's Day, and Father's Day.

Patient Ratios

The typical assignment features a 20-bed unit, collaborating with a Social Worker. During crisis staffing situations, the RN Case Manager may support approximately 1.5 to 2 units in conjunction with Case Management and Social Work departments.

Experience Requirements

• A minimum of 2 years of hospital inpatient RN Case Management/discharge planning experience is required.
• Experience in Med-Surg or Critical Care discharge planning is essential.
• Candidates must be able to work independently after a brief orientation period.
• Experience coordinating discharge needs and transitions of care is crucial.

Certifications

Current Basic Life Support (BLS) certification is required for this position.

Licensure Requirements

An active Registered Nurse license in Maryland must be held upon submission of the application.

Unit Information

This role encompasses responsibilities in Case Management, with a focus on adult and geriatric patients, primarily dealing with discharge planning and patient transitions.

Charting System

The position utilizes the Cerner charting system for documentation and patient care management.
