Registered Nurse (RN) – Acute Care Case Manager
Facilities: Petaluma Valley Hospital and Healdsburg Hospital
Shift and Schedule
- Day shift: 8:00 AM – 4:30 PM
- 5 days per week, 8-hour shifts.
- Every other weekend required.
- Must float between both facilities, approximately 30 minutes apart.
- Weekly schedule may vary between locations depending on staffing and PTO.
Required Experience and Skills
- Minimum 2 years of experience listed in job qualifications.
- At least 1 year of acute-care hospital Case Management experience is a hard requirement.
- Epic experience required — no exceptions.
- InterQual criteria experience required.
- Active Compact RN license must be in hand at submission.
- Travel experience required.
- Experience with care coordination, admission criteria, discharge planning, and CMS guidelines.
Patient and Unit Details
- Adult and geriatric patients.
- Daily census: approximately 30–35.
- Patient ratio listed as 1:15.
- Interdisciplinary support includes PT, respiratory therapy, radiology, social services, pharmacy, transportation, and unit secretary.
Holiday Requirements
Must commit to working 3 of these 5 holidays:
- Thanksgiving
- Christmas Eve
- Christmas Day
- New Year's Eve
- New Year's Day
